Connect clinical information across an evolving care network

A regional care network needs a dependable way to exchange information as providers, systems and pathways change. This platform design brings clinical interfaces, identity reconciliation, terminology services and exchange monitoring into a shared operating model. Its delivery scope covers migration from existing interfaces, parallel validation and staged onboarding of hospital, laboratory and community partners.

The platform accepts authorised clinical events through agreed interfaces, validates their structure and reconciles supported identifiers and terminology. Unresolved records enter an accountable review queue. During migration, old and new routes run in parallel where feasible, with reconciliation reports informing each cutover decision. Integration teams receive a shared service catalogue, replay controls and runbooks for recovering interrupted exchanges.

01
Why This Project Matters

A referral can leave one system without becoming available to the team expecting it. Differences in message formats, patient identifiers, and local codes make these gaps difficult to investigate when each interface has its own logs and support process.

Bringing delivery status and exception handling together helps teams locate the problem and assign responsibility. The emphasis is on making exchanges visible and recoverable while preserving the context attached to the original information.

02
What the Interoperability Hub Does

The hub receives messages through agreed source-system interfaces, checks required fields, and maps supported information into the format expected by the receiving system. Messages with unresolved identifiers or unsupported values are held for review.

Valid messages move to authorized destinations, where acknowledgements and delivery status are recorded. An operations dashboard brings together queue age, processing errors, and retry history so support teams can investigate interruptions and replay eligible messages with duplicate protection.

Project Features

Clinical exchange services

Reusable adapters and agreed data profiles connect existing records without assuming every provider uses the same system.

icon
Identity and terminology stewardship

Assigned reviewers resolve identity conflicts and manage versioned mappings across participating organisations.

icon
Legacy migration and cutover

Interface inventories, reconciliation and rehearsed rollback support a controlled transition between exchange routes.
