Preserve clinical context when care crosses borders

Care across borders depends on more than transferring a document. This platform design connects participating providers through authorised summary requests, controlled terminology services and source-aware clinical views. The programme includes localised workflows, partner conformance testing and staged activation of approved connections, keeping the origin and limitations of each summary visible to the receiving team.

An authorised request triggers identity, purpose and access checks before supported information is assembled into a patient summary. The viewer preserves the original clinical narrative and source timestamps while displaying approved terminology mappings and unresolved items. Danish, Swedish and English interface content follows a controlled localisation process. Each partner connection is tested in an isolated environment before activation; national exchange connections require separate access arrangements.

01
Why This Project Matters

A patient summary from another organization may use unfamiliar codes, different identifiers, or information recorded at different times. These differences can make it difficult to understand what a summary includes and which details require clarification.

Keeping source information and unresolved terms visible helps clinicians assess what they are reading. Access permissions and exchange history also give participating organizations a way to account for requests and the information shared.

02
What the Patient Summary Gateway Does

An exchange begins with an authorized request and checks against the agreed identity, purpose, and access rules. The gateway retrieves permitted information, checks the summary structure, and applies approved mappings to supported coded terms.

The viewer displays the summary with timestamps, source references, and indicators for missing or outdated information. Unresolved terms enter a review queue. Connections to national exchange services depend on the necessary agreements and approvals; automatic translation of clinical free text is outside the scope.
